Swing Bed Hospital Directory

The Social Security Act permits certain small, rural hospitals to enter into a “swing bed” agreement, under which the hospital can use its beds, as needed, to provide either acute or skilled nursing facility (SNF) care. This is a great option for your patient to receive high-quality, post-acute care in a facility closer to home. If you are looking to transfer a patient to a SNF bed, you can find a swing bed provider below. Use the options to filter based on your patient’s individual needs.

Swing Bed eligibility: Patients are eligible for swing bed services if their inpatient hospital stay is at least three days. The majority of patients only stay at a swing bed hospital for 2 – 3 weeks; after 5 weeks, transfer to a long-term care hospital or home care is often considered.  Patients must also meet qualifying medical criteria, including daily therapy and/or rehabilitation, intravenous (IV) therapy, specialized wound care, and pain management. Patients must also be enrolled in Medicare Part A, or other insurance plan that covers swing bed care costs. Click here for more information. If you do not qualify for a Swing Bed, you may be eligible for admission to a long term acute care hospital (LTAC).

This tool provides a general overview of the services available for the hospitals’ swing bed program. Some services are approved on a case-by-case basis. Please reach out to the listed contact for specific details on your patient’s need.
