During this session, the speaker will discuss the impact of the updated CMS HCC Model (which was fully implemented for 2025 dates of service), the importance of comprehensive and accurate documentation and ICD-10-CM coding in supporting accurate patient risk scores under the current model, and strengthening HCC coding engagement with both providers and coding professionals.
