The Kentucky Hospital Association Quality Team just announced an opportunity for you to nominate someone you know for the KHA Patient Safety Hero Award. This award raises awareness and honors innovation in a hero’s commitment to highly reliable, patient-centered care. Rewarding successful efforts of individuals who develop and promote improvements in quality of care can inspire organizations to systematically integrate and align their quality improvement performances throughout the Commonwealth.

In order to nominate your patient safety hero, who may be any clinical or non-clinical staff employed within a KHA member hospital, complete the nomination form.
