SUMMARY:KHA Quality Webinar: Peer-to-Peer Learning
DESCRIPTION:Please join the KHA quality team and Saint Joseph Hospital to take a deep dive into their journey of quality improvement project work to achieve excellence in patient care and outcomes. Please read more below for a detailed look at their project and what exciting information you will learn! \n\nKHA MEMBERS: CLICK HERE TO REGISTER\n\nSaint Joseph Hospital: Preventing the Preventable: Enhancing VTE Prophylaxis in Hospital Settings Through Pharmacy-Driven Intervention \nMonica Owen\, PharmD\, BCPS\, BCCCP\, Clinical Pharmacist Specialist and April Jennings\, DNP\, RN\, CPHQ\, HACP-CMS\, Director of Quality and Patient Safety from Saint Joseph Hospital will be presenting on their goal to enhance Venous Thromboembolism (VTE) prophylaxis in the hospital setting. This project represents a significant shift from a reactive to a proactive system-level approach for VTE prevention\, driven by real-time EHR data. By empowering clinical pharmacists with a standardized workflow and leveraging EHR reports for precise identification of at-risk patients. The significant improvement in VTE prophylaxis rates and the observed reduction in PE NPOA underscore the project’s clinical efficacy. This proven practice laid a strong foundation for permanent adoption across Saint Joseph Hospital and for widespread implementation within other CommonSpirit Health facilities in the South Region\, ultimately extending its positive impact on patient safety and VTE reduction on a broader scale. \n  \n

SUMMARY:KHA Quality Webinar: Performance Improvement Tool Series Part 3: From SMART Goals to Success - Implementing & Sustaining Quality Improvement
DESCRIPTION:KHA MEMBERS: CLICK HERE TO REGISTER\n\nIn this final installment of the S.M.A.R.T series\, the KHA Quality Team will move from planning to action by exploring how S.M.A.R.T goals and Performance Improvement (PI) tools work together to drive meaningful\, lasting change. While S.M.A.R.T goals define what we want to accomplish\, PI tools guide us in how to achieve those outcomes through clear data\, structured analysis\, and focused interventions. But the true challenge and greatest opportunity lie in implementation and long‑term sustainability. \nAs organizations continue embedding S.M.A.R.T goals and PI tools into their daily practice\, improvement becomes not a one‑time project but a continuous cycle. This approach ensures that gains are not only achieved but maintained\, scaled\, and woven into the culture of excellence that our patients deserve. Registration is required for this webinar\, and the registration link is provided below. \nLearning Objectives: \n\nUnderstand how SMART Goals and PI Tools Complement each other to define objectives and guide effective PI efforts.\nIdentify strategies for implementing and sustaining improvement initiatives using structured data\, analysis\, and focused interventions.\nRecognize the importance of embedding continuous improvement into daily practice to maintain gains\, scale success\, and strengthen a culture of excellence in patient care.\n\nSpeaker: Billie A. Delauder\, DNP\, MSN/RN\, CPHQ\, CPPS\, HACP-CMS\, CFPS\, Quality Process Improvement Specialist\, Kentucky Hospital Association \nTarget Audience: Health care leaders\, quality/performance improvement teams and leaders\, patient safety officers\, risk management professionals\, clinical leadership\, physician and nursing leaders\, clinical educators training around quality and safety\, and other Health Care Professionals interested in quality and performance improvement.
