Participants will walk away with real-world strategies\, lessons learned\, and actionable ideas they can adapt within their own organizations to improve patient safety and outcomes. Together\, both presentations will demonstrate how health care organizations achieve meaningful improvements in both patient safety and operational efficiency through collaboration\, innovation\, and a commitment to continuous improvement. \nBaptist Health Paducah implemented a multidisciplinary daily surveillance huddle to proactively review central lines and indwelling catheters. Nursing leaders\, Infection Prevention\, Vascular Access\, and education staff work together to assess medical necessity\, identify high-risk patients\, provide real-time coaching\, and address potential safety concerns. This collaborative approach has strengthened communication\, accountability\, and infection prevention practices across the hospital. \nUK King’s Daughters successfully reduced CLABSI events through a proactive\, multidisciplinary approach focused on evidence-based practices\, staff education\, standardized processes\, and bedside engagement. By using data to identify risks and opportunities\, the team strengthened collaboration\, accountability\, and a culture of prevention to improve patient safety.
