SUMMARY:Root Cause Analysis and Action (RCAA) Learning Series\, Part 3: The Second “A”
DESCRIPTION:KHA MEMBERS: CLICK HERE TO REGISTER\n\nWhen a serious safety event or near miss occurs\, an effective Root Cause Analysis and Action (RCAA) process can be the difference between a missed opportunity and meaningful\, lasting improvement. Yet many health care leaders struggle to conduct RCAAs that move beyond compliance and drive real change. \n\nJoin the KHA Quality team for a four-part learning series designed to help hospital leaders build confidence and competence in every stage of the RCAA process. From understanding the fundamentals to developing impactful action plans and participating in a realistic tabletop exercise\, this series will equip attendees with practical tools they can immediately apply within their organizations. \nParts 1–3 will feature instructor-led education presented by KHA Associate Vice President of Quality and Health Professions Casey Franklin\, BSN\, RN\, CPHQ. Part 4 will be co-led by Casey Franklin and KHA Associate Vice President and Associate General Counsel Kristen Fowler\, and will provide participants with an immersive\, hands-on RCAA simulation. \nRoot Cause Analysis and Action (RCAA) Learning Series\, Part 1: The Basic Components\nThursday\, July 16   ||   1:00–2:00 p.m. ET \nBuild a strong foundation by learning the essential elements of an effective RCAA. Participants will walk through each step of the process — from event identification and investigation to action planning and follow-up — gaining a practical framework for conducting RCAAs that lead to meaningful improvement. \nRoot Cause Analysis and Action (RCAA) Learning Series\, Part 2: From Good to Better to Best\nThursday\, July 30   ||   1:00–2:00 p.m. ET \nDiscover what separates a routine RCAA from a truly impactful one. This session explores common pitfalls\, ineffective habits\, and proven strategies that help organizations uncover deeper system issues\, strengthen safety culture\, and maximize the value of every RCAA. \nRoot Cause Analysis and Action (RCAA) Learning Series\, Part 3: The Second “A”\nThursday\, August 13   ||   1:00–2:00 p.m. ET  \nA strong analysis is only valuable if it leads to effective action. Learn how to develop action plans that address root causes\, drive sustainable change\, and produce measurable improvements in patient safety and organizational performance.

SUMMARY:August Sepsis Case Abstraction Office Hours
DESCRIPTION:The Kentucky Hospital Association (KHA) Sepsis Consortium’s Sepsis Case Abstraction Office Hours will continue in August\, with the next session scheduled for Wednesday\, August 26\, 2026\, from 11:00 a.m.–12:00 p.m. ET. These collaborative sessions support sepsis abstractors in accurately identifying and abstracting CMS SEP-1 cases involving Severe Sepsis and Septic Shock—critical components of patient safety\, regulatory compliance\, and appropriate reimbursement. \nDr. Billie Delauder will bring her CMS sepsis abstraction expertise and\, together with Deb Campbell\, Vice President of Clinical Strategy and Transformation\, will provide guidance\, answer practice-focused questions\, and review case rationales and applicable guidelines. Participants will also have the opportunity to submit questions directly to CMS Quest. \nQuality leaders and sepsis abstractors are encouraged to attend and are welcome to bring case-specific questions. \nPlease note: No Protected Health Information (PHI) will be shared during this event. \nFor our member hospitals: Invitations will be sent to existing members of the event. However\, if you or someone you know would like to be included in future invitations\, please contact Billie Delauder at bdelauder@kyha.com.

SUMMARY:KHA Quality Webinar: Peer-to-Peer Learning- Two Hospitals. Two Strategies. One Goal: Preventing Patient Falls
DESCRIPTION:Join the KHA Quality Team for an engaging Quality Webinar on November 2\, 2026\, from 1-2 PM ET featuring two Kentucky hospitals that successfully implemented innovative fall prevention strategies tailored to their unique patient populations. While every health care setting faces different challenges\, the shared goal remains the same — keeping patients safe while fostering a culture of continuous learning and improvement. \n\nKHA MEMBERS: CLICK HERE TO REGISTER\n\nRepresenting the acute care setting\, UofL Health will highlight its innovative “Debrief after the Debrief” process\, a secondary post-fall review that transforms fall events into valuable learning opportunities. This interdisciplinary\, just culture approach strengthened accountability\, improved documentation\, increased staff engagement\, and helped reduce falls from 5.8 to 4.3 falls per 1\,000 patient days. \nRepresenting the Long-Term Acute Care Hospital (LTACH) setting\, CommonSpirit – Continuing Care Hospital – Lexington will share how participation in a system-wide fall prevention initiative led to standardized practices\, enhanced staff education\, stronger leadership engagement\, and improved interdisciplinary collaboration. These efforts reduced falls from 3.5 to 2.2 falls per 1\,000 patient days while reinforcing a culture of safety. \nWhether you work in acute care\, critical access\, LTACH\, rehabilitation\, or quality improvement\, you’ll leave with practical\, evidence-based strategies you can adapt within your own organization. Learn how leadership support\, frontline engagement\, standardized processes\, and a focus on learning — not blame — can create lasting improvements in patient safety.  \nTarget Audience: Chief Nursing Officers (CNOs)\, Chief Quality Officers (CQOs)\, Medical Directors\, Patient Safety Officers\, Risk Management Professionals\, Quality and Performance Improvement Teams\, Hospitalists and Inpatient Physician Leaders\, Nursing Directors and Nurse Managers\, LTACH and Rehab Leaders\, ED Directors\, Managers ED Physicians\, and ED Nurses\, Care Coordination and Case Management Leaders\, Patient Care Services Leaders\, Clinical Practice Leaders\, Clinical Educators\, and other health care professionals.
