DESCRIPTION:Join this KHA Quality Webinar to learn more about addressing safety for frail patients. \nThe Age Friendly Structural Measure was added to the Centers for Medicare & Medicaid Services (CMS) Final Rule on August 8\, 2024\, and will start with the calendar year (CY) 2025 Reporting Period\, affecting fiscal year (FY) 2027 payments. Domain 3 focuses on Frailty Screening & Intervention\, requiring hospitals to screen for cognitive impairment/delirium\, physical function/mobility\, and malnutrition for early detection and intervention. Guided by the Institute for Healthcare Improvement (IHI) Age Friendly 4Ms (What Matters\, Medication\, Mentation\, Mobility)\, this measure addresses the interconnectedness of these elements.
