1) CMS VALIDATION OF MEDICARE SS# and HIC#

During the CMS validation process if a patient’s social security number or HIC number (entered during CART abstraction) does not find a matched SS number or HIC number in the CMS Medicare data, the patient case will be rejected from the QIO warehouse. CART abstractors are cautioned to verify data entry of each patient’s SS# or HIC# to ensure it is entered correctly. Please note COMPdata does not include a patient’s SS# or HIC# from the UB/Administrative data. CMS will post hospital error reports on the CMS QNET Exchange private web site for hospitals to review each quarter.

 2) REVIEW PERFORMANCE MEASUREMENTS DATA COLLECTION FEEDBACK REPORTS

The COMPdata Performance Measurements Data Collection Feedback Reports are designed to help hospitals ensure clinical data submitted by your facility was received, processed and merged with the UB data submitted. For your review, the following is an explanation of the data collection feedback reports for Core:


 Email Notification Reports Available

After each submission of clinical data to COMPdata (via EFT), hospital contacts receive an email notification advising performance measurements data collection feedback reports are available for you to access. Since COMPdata processes data every 15 minutes, you should receive notification within 8 hours that your feedback reports are available. If you do not receive the email notification, please contact COMPdata at perfmeas@ihastaff.org

Acknowledgement Report:

This report identifies the number of successful cases processed at COMPdata for each measurement topic submitted (by receive date) as well as cases that failed to process. This report provides the individual file name that failed with an explanation of failure. The following are reasons a case may fail to process at COMPdata:

Field Null When Info Required (e.g. discharge date blank)
Field Size Too Large
Invalid Number
Invalid Medicare Provider ID
Quarter Closed
Invalid Date Format
Case Status – Record Error
Case Status – Not Eligible
Case Status – Edit Errors
Multiple Admit or Discharge Dates
Incorrect file version (CART version)

Missing Clinical Data Report

This report provides a listing of UB/Administrative cases received at COMPdata where a clinical case is not found that matches on three key fields, birth date, admission date or discharge date. COMPdata generates UB/Admin data according to each performance measurements population eligibility. You may find many UB/Admin cases listed on the Missing Clinical Data Report that will never have a clinical case submitted due to exclusions for that measure.  This report allows hospitals to ensure all possible clinical cases are indeed abstracted for each quarter. You should compare this report to the UB/Admin Missing Data Report to identify cases possibly miskeyed. Please be sure to correct and resubmit miskeyed cases to COMPdata.

Missing UB/Administrative Data Report

This report provides a listing of Clinical cases received for a quarter where there is no UB/Admin case that matches on the three key fields, birth date, admission date or discharge date. Please contact your UB/Administrative data coordinator and ask if the UB/Admin record was submitted to COMPdata. Ask if the UB/Admin record was accepted or rejected at COMPdata. Records that fail the UB/Admin edits are not accepted in the COMPdata database and must be corrected and resubmitted.

Matched Cases Report

This report contains all cases that have a clinical and UB/Administrative merge/match.  Cases listed on this report will be used at quarter close to calculate the algorithms for JCAHO; the actual XML matched files will be submitted to CMS QNET Exchange.
